I think it's useful for even completely physically healthy people who are in a position that overloads them. They just start out with more spoons and get them back faster, so they have to be put under (generally) more strain before starting to run out.
Neither have I, but I've known people who were. I'm an effort-induced asthmatic with allergies and a few other issues (new ones as I've gotten older). Still, even the healthiest people in the world can get overloaded. They just start out with three cases of spoons and get a full new case every day, while people like me start out with one case of spoons and get maybe half a case each day, and people with much more serious issues start out with maybe a dishwasher utensil rack of spoons and get two or three back per day.
