CPAP Place has since rediscovered the forms and are in the process of sending them. Whether e-mail or paper-mail has yet to be confirmed.

Dental cleaning tomorrow. I have done without this for over a year, the dentists' office told me. We'll see how much that matters. Probably more than I'd like.

It's not the cleanings themselves that grate. It's the consequences of what else might be noticed and have to be dealt with.
