Definite failure to test with multiple browsers, which means ignorance and failure in IT procedures.
When we brought up a new content management system for the city I worked for a decade ago, I tested it ruthlessly with three different browsers on my Mac to make sure it rendered properly since we were a Windows shop. And I caught some things that were subsequently fixed. I don't recall if anyone tested it with linux.
I know of at least one federal agency that tests in multiple OS iterations of Mac and Windows and multiple flavour of Linux, as well as on cellphone-based web browsers. It is not Service Canada, though.
